Please fill out our “Consultation Request Form” and fax it to our office, or call one of the offices listed.
Please call (888) 931-0077. If you have an emergency after hours, one of our doctors will always be on call to assist you.
Please ask the patient to complete and sign the “Authorization for Release of Information” form and fax it to our office.
As the COVID-19 situation has evolved, PRC has revised and updated its patient care procedures.
- Wear a mask to clinic for your visit.
- Reschedule your visit if you are symptomatic.
- Check in as close as possible to your scheduled time, to minimize your time in proximity to others.
- Drivers are asked to wait outside of clinic, and to be accessible by cell phone when your care is concluded.
