Interested in connecting with us? We appreciate the trust you’re placing in our team and hope to make the first steps easier by answering some key questions here.

  • Please fill out our “Consultation Request Form” and fax it to our office, or call one of the offices listed.

  • Please call (888) 931-0077. If you have an emergency after hours, one of our doctors will always be on call to assist you.

  • Please ask the patient to complete and sign the “Authorization for Release of Information” form and fax it to our office.

  • As the COVID-19 situation has evolved, PRC has revised and updated its patient care procedures.

    • Wear a mask to clinic for your visit.
    • Reschedule your visit if you are symptomatic.
    • Check in as close as possible to your scheduled time, to minimize your time in proximity to others.
    • Drivers are asked to wait outside of clinic, and to be accessible by cell phone when your care is concluded.

  • Our main office is in West Columbia, SC, and we also have offices in Columbia, Orangeburg, Sumter, Florence, and Aiken as well. Find the office closest to you for your next appointment here.
